Care Management:
Needs assessments, case management, service information, direct referrals, linkage to other resources.

Caregiver Support Services:
Private consultations, consumer materials, direct referrals to community resources for caregivers of older adults.

Friendly Visiting:
Regular visits to homebound older adults.  Heritage Place is the largest Senior Companion Station in central Indiana.  Each week, over 35 senior companions provide 20 hours of friendly visiting services.  Less frequent friendly visiting is also provided by community volunteers.      

Home & Yard Work:
Light house cleaning and seasonal yard work.

Senior Center:
A wide assortment of educational classes, legal and financial information, health services and social events.  We collaborate with over 50 entities, adding depth and variety to our services.

Transportation:
Referrals for non-emergency transportation.
